Traditional Asian Rhinoplasty Surgery

Rhinoplasty surgery for the Asian patient can be one of the most complicated procedures a rhinoplasty surgeon performs. Surgically changing the appearance of the Asian nose in a way that does not compromise the nasal structure and looks natural and subtle is not easy. The downtime, surgical risk, and expense of the procedure are considerable and the possible side effects are severe. Dr. Rivkin has treated many Asian patients who had received silicone implants as part of their surgery to raise their bridge. After the procedure, their implants became infected and had to be removed, leaving scarring and the same low bridge that they were trying to correct. They decided on Non-Surgical Rhinoplasty because operating on their nose a second time would have been much riskier and significantly less likely to produce a natural-looking result. Dr. Rivkin developed and perfected the Asian Non-Surgical Rhinoplasty procedure to address the particular concerns of his Asian clientele. Dr. Rivkin is also the world’s expert on Non-Surgical Rhinoplasty. He has made this procedure his specialty and he has performed more of them over his twelve years in practice than anyone else.

Non-Surgical Rhinoplasty is not a substitute for surgery and is not a procedure that will be appropriate for all patients. During the consultation, Dr. Rivkin will examine you and together you will decide if your goals can be met with the Non-Surgical Rhinoplasty procedure or if surgery would really be your best option.

Treating the Concerns of Asian and Asian-American Patients

About one-third of all Non-Surgical Rhinoplasty procedure clients Dr. Rivkin sees is Asian and he has tailored this variation of the procedure especially for them.

Dr. Rivkin explains, “The most common request we receive from our Asian patients is to raise and better define a low or wide nasal bridge. Clients come in saying that their glasses rest on their cheeks, that the tip of their nose looks too prominent, and that their face lacks balance because the nose is not well defined. A low nasal bridge emphasizes and brings attention to the nasal tip, making the tip appear larger than it really is. When the bridge is higher and better defined, the tip is no longer the major visible part of the nose. A well-defined bridge brings the tip into better proportion with the rest of the nose and makes it look smaller because the tip is no longer attracting as much attention. A defined bridge also makes the nose look stronger and more substantial, thereby making the face more proportional and symmetric. Finally, augmenting the nasal bridge permits sunglasses to rest on the nose instead of the cheeks.”

Providing Better Definition to Bridge and Tip

Many of our Asian clients also request better definition and less roundness to the tip of their nose. Some want their tip to be smaller. Changing the size and definition of the tip is not easy, but can be done in most cases. The challenge of improving the definition of the tip is to use filler very carefully so that there is no overall increase in size. Cortisone injections also help in reducing the size of the tip and making it appear less rounded and more square. Dr. Rivkin uses concentrated Kenalog mixed with 5-FU and Dexamethasone to improve the effect. The results of steroid injections are somewhat variable, but we do see benefits for most people. Another frequent request is to lower the nostrils so that the nostril openings are not so large and visible. This is possible using small amounts of filler in the proper areas.

The third area of concern for Asian clientele is the radix or very top of the nasal bridge. When it is low and not well defined, the complaint is that the nose looks short and the eyes look far apart. This is easily remedied using a filler injection to raise the bridge at that point.

Deciding on the Type of Filler

Non-Surgical Rhinoplasty can be performed using a variety of fillers. Dr. Rivkin usually uses Voluma and Radiesse as the temporary fillers and Bellafill (formerly known as Artefill) as the permanent filler. He also sometimes uses Restylane Lyft (formerly known as Perlane). Lyft and Voluma are ideal for clients that are hesitant about the procedure because these hyaluronic acid fillers can be dissolved anytime with a simple enzyme injection. For most Asian Non-Surgical Rhinoplasty clients, Dr. Rivkin prefers to use Voluma or Radiesse at first and then switch to Bellafill after a year and a half to two years, when the temporary fillers have dissolved.

Unlike the other fillers, Bellafill requires a skin test a half hour before the procedure to make sure that you are not one of those very rare people who are allergic to the bovine collagen component of the filler. The procedure itself is the same as what is done with Voluma or Radiesse, but Bellafill settles somewhat after injection, so we usually need an average of three sessions to achieve the desired permanent effect.
